Nurse Practitioner - Pediatric Oncology (Full-Time, Day Shift)
The Nurse Practitioner for Sutter Children’s Center-Inpatient Services provides nurse practitioner and case management services for infant, child, and adolescent patients. The NP provides patient assessments, patient/family education, clinical case management and promotes continuity of care throughout the continuum. The incumbent participates in daily inpatient rounds, case conferences, program planning, program evaluation and process improvement activities. The incumbent also assists with clinical effectiveness/research studies. Education/Certification/Licensure:Bachelor's Degree in Nursing is required.Master's Degree is required.Pediatric Nurse Practition experience is required.PALS certificaiton is required within 6 months of hire. PEDIATRIC ONCOLOGY CLINICAL CAREProvides direct services to pediatric patients with cancer through physical assessments and development of care plan, as well as assessment of patient and family education needs related to medical issues.Follows patient progress providing clinical support to alleviate identified concerns.Develops innovative strategies for managing complex clinical problems.Participates in case conferences.Assures accurate completed documentation including national cooperative group protocol requirements.Coordinates care with good understanding of national cooperative group protocol requirementsAssists staff in utilizing available clinical resources. PROVIDES PATIENT CARE COORDINATIONEnsures accurate and comprehensive communication to core team regarding patient findings, progress and concerns and promotes cohesive team approach to problem resolution.Serves as clinical case manager for assigned team patients and assures follow through with unit-specific case management policy and procedures.Discusses patient care with family, health professionals and outside agencies, assess understanding and re-educates or modifies intervention as necessary to facilitate follow through with specialty care team recommendations.Ensures the continuity of care throughout the continuum of health care through established communications practices. Accurately documents patient care coordination activities.
